Prairie View, is seeking a dedicated, compassionate, and certified Care Coordinator to join our team in Hillsboro. In this role, you will act as a vital support system and advocate, helping children with high behavioral and emotional needs navigate their care, build on their strengths, and successfully integrate into their schools and communities. Clinical Care Coordination Relationship Building:
Develop and maintain supportive, ongoing relationships with young clients and their caregivers using strengths-based interventions.
Treatment Planning:
Facilitate initial treatment planning meetings and 90-day reviews. Partner closely with therapists to update goals, objectives, and services as clinically indicated.
Family Coaching & Advocacy:
Assess family strengths and needs, interpret service options, and provide family coaching, education, and community service navigation.
Crisis & Safety Support:
Coordinate crisis intervention services in collaboration with care team members, maintaining an acute awareness of safety risks and appropriate interventions. 2.
Systems Collaboration & Administration Teamwork:
Collaborate with multi-disciplinary teams and external partners, including physicians, schools, therapists, and community providers.
Waiver Management:
Update Plans of Care for Severe Emotional Disturbance (SED) waiver patients (30 days post-start, and every 90 days thereafter) and coordinate with Utilization Review for service authorizations.
Timely Documentation:
Maintain exceptionally accurate records. Our team standards include completing 90% of progress notes within 72 hours, completing waiver documents/budgets within two weeks of a case conference, and submitting service verifications within 72 hours. 3.
Professional Growth & Community Connection On-the-Job Mobility:
Travel locally to meet clients where they are, ensuring reliable transportation and community-based support.
Continued Learning:
Partner with your supervisor on an annual professional development plan, utilizing Essential Learning resources and attending specialized workshops or seminars.
Position Requirements Experience & Education Youth Focus:
A minimum of two (2) years of experience working directly with children or adults. Experience working specifically with populations with Severe Emotional Disturbance (SED) and their families is highly preferred.
Age Requirement:
Must be at least 21 years of age.
Licensure & Certifications Case Management:
Must currently hold and maintain a State of Kansas Case Manager Certification and keep up with required Continuing Education Units (CEUs).
Driver's License:
Must hold a valid Kansas driver's license, pass a driver's license verification check, and maintain current personal automobile insurance.
Skills & Background Check Communication:
Strong listening, verbal, and written communication skills, with the ability to observe and assess patient demeanor and non-verbal cues.
Flexibility:
Basic computer proficiency and a willingness to work flexible hours based on patient and agency needs.
Safety Clearances:
Must successfully pass a Kansas Bureau of Investigation (KBI) background check, a Central Registry Screen, and a Department for Children and Families (DCF) background check. About Prairie View, Inc.
