Full Job Description Now Hiring:
Care Coordinator Full Time | $18 to $20 per hour Phoenix Home Care & Hospice is looking for an organized and dependable Care Coordinator to support the daily operations of our home care services. In this role, you will help ensure caregivers are scheduled appropriately and clients receive the care they depend on. Care Coordinators play an important role in keeping care running smoothly by supporting field staff, communicating with clients and families, and maintaining accurate scheduling and documentation. If you thrive in a fast-paced environment, enjoy problem solving, and take pride in helping others, this could be a great fit.
