Care Coordination & Planning Develop, implement, and update individualized care plans based on resident assessments Coordinate services among nursing staff, Monitor residents' progress and adjust care plans as needed Staff Support & Communication Serve as a liaison between residents, families, and healthcare team members Provide guidance and support to caregiving staff to ensure quality service delivery Participate in care team meetings and case reviews Compliance & Documentation Ensure accurate and timely documentation in accordance with regulatory requirements Maintain compliance with federal, state, and facility policies and standards Resident & Family Relations Communicate care plans and updates with residents and families Address concerns, complaints, and provide resolutions in a timely manner Quality Assurance Assist in monitoring quality of care and implementing improvements Participate in audits, surveys, and inspections Charter Senior Living is dedicated to the care and wellbeing of our residents.
