Job Description

Always Best Care Senior Services | Full-Time Join a team where responsiveness, compassion, and strong follow-through directly improve the lives of seniors and their families. Always Best Care Senior Services is seeking an experienced, highly organized Care Coordinator to manage client relationships, support new client intake, coordinate care services, and help resolve issues in a fast-paced home care environment. This is not a strictly desk-based position. The Care Coordinator communicates with clients, families, caregivers, referral partners, and internal departments throughout the day; conducts occasional field visits and reassessments; participates in community events; and shares rotating after-hours call responsibilities. What You Will Do Handle incoming inquiries from prospective clients and families by phone, email, and text, provide accurate information about services, and maintain timely follow-up throughout the intake process. Respond promptly to client, family, caregiver, and internal team communications while maintaining a high level of professionalism and urgency. Review overnight and after-hours reports each morning, identify urgent concerns, complete necessary follow-up, and help resolve issues before they affect service continuity. Build and maintain strong relationships with current clients through proactive communication, service-satisfaction follow-up, and discussions about changing care needs or additional service hours. Coordinate with staffing team members to support successful starts of care, appropriate caregiver placement, and a strong caregiver-client match. Complete and update plans of care based on assessments, client needs, family communication, and changes in condition or service requirements. Conduct occasional client visits, drop visits, and reassessments to monitor service quality, compliance, and the appropriateness of the care plan. Handle incident reports, client or caregiver concerns, urgent situations, and day-to-day problem resolution while protecting client safety and continuity of care. Review client visit documentation and coordinate with Client Service Associates regarding caregiver coaching or training needs. Follow up with hospitalized clients, assist with continuity-of-care planning, and complete discharge documentation and exit interviews when services end. Coordinate with billing staff to address client invoice questions and communicate changes that may affect billing or service authorizations. Prepare assigned information for monthly reports and participate fully prepared in Monday and Friday interdisciplinary team meetings and monthly PACE partner meetings. Maintain accurate, timely, and thorough documentation in WellSky and other required company systems. Participate in occasional community outreach, referral relationship-building, and marketing events. Provide after-hours call coverage every other week and respond to urgent client and caregiver needs outside normal business hours. What We Are Looking For College degree required; a degree in social work, healthcare, human services, business, or a related field is preferred. Three to five years of experience in a fast-paced office, client services, case management, healthcare, home care, senior care, or a related environment is preferred. Experience handling client intake, assessments, care coordination, case management, or service recovery is strongly preferred. Excellent phone, written, and interpersonal communication skills, with the judgment to adjust communication style based on the individual and situation. Strong organization, prioritization, time-management, and follow-through skills. Ability to remain calm, make sound decisions, and respond quickly when unexpected or urgent situations arise. Comfort managing multiple priorities, frequent interruptions, and time-sensitive responsibilities without losing attention to detail. Intermediate to advanced proficiency with Microsoft Outlook, Word, and Excel. Experience with WellSky or similar home care scheduling or client-management software is preferred. Ability to maintain confidentiality and comply with company policies and applicable Louisiana Department of Health and other regulatory requirements. Valid driver's license, reliable transportation, and willingness to travel locally for client visits, reassessments, meetings, or community events. Availability to participate in the rotating after-hours on-call schedule. Position Details Full-time position. Primarily Monday through Friday, with a combination of office and local field responsibilities. Rotating after-hours call coverage every other week. Occasional evening or weekend meetings, training, or community events may be required with advance notice when possible. Why Join Always Best Care?
