Position Summary MPCS is seeking Care Coordinators to join our growing team. The Care Coordinator is a vital role on the MPCS team and serves as the first point of connection with our clients and oversees all aspects of assessing their needs and connecting them with resources both within MPCS and in our community. Key aspects of the role include: Assess needs of clients, family members, and caregivers in a conversational setting employing a personal approach in a caring, respectful, and non-clinical manner. Respond to the immediate needs of clients, family members, and caregivers respectfully and compassionately. Document all communications with clients, family members, and caregivers in the MPCS client database. Promote and provide timely information on programs available to clients, family members, and caregivers via written or verbal means. Serve as a project coordinator for assigned internal and external projects that support our clients and community. Assess effectiveness of services and programs offered and continually look for ways to improve and expand service and program offerings. Provide coverage of client-related events held on a monthly basis both during and after business hours (evenings and weekends). This includes, but is not limited to Support Group Night and SMILES for Miles. Seek to stay informed of cancer related news and maintain awareness of community resources that could impact service offerings as well as benefit clients, family members, and caregivers. Remain sensitive to client, family member, and caregivers' emotional needs as they deal with cancer diagnosis, treatment, and recovery. Perform all job responsibilities at the MPCS office and on Mobile Miles, a van which extends services to outlying communities throughout Acadiana. (Actual assignments will be determined by the Director.) Other duties as assigned by the Director of Services. Qualifications Experience in case management in a non-profit or social services organization required. Bachelor or Master of Social Work degree required. Experience working in oncology and with clients of all ages is preferred. Leads with a servant heart and empathy toward self and others, while maintaining healthy and professional boundaries with clients and their families. Self-starter with strong teamwork, interpersonal, and active listening skills. Highly organized and detailed oriented. Ability to connect to people of diverse backgrounds. Highly effective communication and interpersonal skills to properly engage clients in stressful situations and medical crises. Ability to maintain focus on multiple projects and appropriately prioritize activities. Ability to work collaboratively and effectively with individuals at all levels of the organization and MPCS's outside partners. Ability to maintain client confidentiality at all times. Must be able to perform physical tasks such as lifting objects weighing as much as 25 pounds, bending, stooping, and reaching. Must be able to drive the MPCS Mobile Miles van, a large passenger van, to the Mobile Miles locations in New Iberia, Opelousas, Crowley, and Mamou.
