Job Description

MDS Coordinator Job Summary:
The Center for Extended Care at Amherst is seeking a dedicated and compassionate MDS Coordinator to join our team. As a key member of our clinical staff, you will play a vital role in ensuring the highest quality of care for our residents. Our family-owned and operated facility has been dedicated to providing exceptional rehabilitative and skilled nursing services since 1967. We are committed to creating a warm and welcoming environment that fosters satisfaction and excellence among our staff and residents alike.
Responsibilities:
Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each resident Conduct comprehensive assessments, including Minimum Data Set (MDS) assessments, to identify resident needs and develop effective care plans Communicate with residents, families, and healthcare professionals to ensure seamless care coordination and excellent customer service Maintain accurate and up-to-date records and databases to ensure compliance with regulatory requirements Participate in case conferences and care plan reviews to promote continuity of care and improve resident outcomes Leverage expertise and knowledge to support staff development and training, promoting a culture of excellence and innovation
Requirements:
MDS Coordinator certification or experience in a similar role Strong understanding of MDS assessment and care planning principles Excellent communication, organizational, and interpersonal skills Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines Strong analytical and problem-solving skills, with the ability to think critically and creatively Familiarity with electronic medical records and computer-based systems
