$50,000 annually
Schedule:
Monday-Friday, 9:00 AM-5:00 PM (40 hours/week)
REQUIRED - READ CAREFULLY
Valid U.S. driver's license is
REQUIRED
Must have reliable access to an INSURED personal vehicle Driving is an essential function of this role (community outreach, provider visits, care transitions) Applicants WITHOUT a driver's license and vehicle will NOT be considered Must be 21+ and able to pass a driving background check Vinfen is seeking a CP Care Coordinator to provide care coordination and care management for MassHealth members with complex medical and behavioral health needs who are enrolled in an ACO or MCO plan. This role focuses on outreach, engagement, care planning, and coordination across medical, behavioral health, and community-based services. Key Responsibilities Conduct outreach and engagement with individuals referred to the Community Partner program Coordinate Comprehensive Assessments, risk assessments, and care plans Lead and support care transitions, including hospital discharges Collaborate with PCPs, specialty providers, hospitals, and community organizations Connect individuals to social services addressing social determinants of health Support "hard-to-reach" individuals through persistent outreach and engagement Serve as a shared resource with ACO/MCO teams and interdisciplinary partners Ensure continuity, quality, and person-centered care Complete required documentation accurately and on time Requirements High school diploma or equivalent required (BA/BS in human services preferred) Minimum of 3 years of care management experience preferred Experience working with individuals with SMI and/or SUD preferred Strong organizational, communication, and problem-solving skills Ability to work independently and as part of a multidisciplinary team Commitment to person-centered, recovery-oriented, and trauma-informed care Certifications (Required within Vinfen timelines) CPR First Aid Safety Care
