Job Description

The Hampden County Sheriff's Office is seeking compassionate, dedicated individuals to join our team as a Care Coordinator in the Medication Assisted Treatment (MAT) Program, primarily at our Main Institution in Ludlow, as well as other of our facilities as needed. In this vital role, the Care Coordinator supports justice-involved individuals on their path to recovery by facilitating educational and treatment groups and assisting with aftercare planning. Through empathy, professionalism, and a commitment to excellence, the Care Coordinator helps foster a safe, structured, and respectful treatment environment while supporting the effective operation of a program that addresses substance use disorder and promotes long-term success.
Work schedule:
Monday-Friday, 9:00 AM to 5:00 PM; must be able to work any shift as directed What You'll Do The Care Coordinator facilitates psycho-educational and treatment groups; observes, engages, and builds therapeutic rapport with participants; and responds appropriately to changes in behavior, including using de-escalation techniques when necessary. This role includes maintaining accurate and timely documentation, participating in treatment and staff meetings, sharing relevant program and community information, and collaborating with Sheriff's Office staff to develop behavior management plans and support a positive treatment environment. The Care Coordinator also provides consultation and education to staff to enhance understanding and continuity of care for program participants. At least two (2) years of clinical experience in the field of substance addiction treatment. High School Diploma or equivalent required;LADC/CAC
preferred. Ability to establish and facilitate a therapeutic rapport with client population, engage in effective individual counseling and intervention. Strong computer skills with working knowledge of Microsoft Office 2010. Strong written and verbal communication skills. Ability to prepare effective, concise, and accurate written reports and discharge plans. Strong crisis intervention skills. Bilingual skills in speaking, writing, and reading English and Spanish are preferred. Valid Driver's License. Maintain known and accessible telephone number.
