JOB SUMMARY
Provides direction and oversight to the Social Work and Outreach Training Program, which includes students and staff at various levels of professional development, including participation in planning continuing education for staff. This position is also responsible for providing direct support and communication to the site medical team as well as a wide range of social work and medical case management services to CBHS patients and families, with the goal of addressing social determinants of health that directly impact access to and engagement in medical care and treatment. Activities include working with an interdisciplinary team to support, guide, and address treatment, education, adherence, and psychosocial needs; directly facilitating specialty medical care referrals, and external medical stakeholder activities/communication as needed.
MAJOR DUTIES AND RESPONSIBILITIES
Performance Management Develop and maintain a curriculum for Social Work and Outreach interns training. Coordinate supervision of trainees with Social Work and Outreach providers. Coordinate the initial training to assess clinical competencies required of all new trainees across all sites. Perform data reporting and chart reviews to identify patients in need of MADAP and Medicaid redetermination across all sites. Monitors ACA activities completed by Certified Application Counselors, Navigators, and Assisters at all CBHC sites and events. Resource, Staff, and Budget Recruit and screen applicants for the student internship programs. Monitor productivity and effectiveness of trainees' schedules and access to meet the needs of the Social Work and Outreach team. Analytical and Critical Thinking Acquire and maintain NASW CE approvals and assist the Director of Social Work and Outreach with developing a CEU program for staff at all levels. Provide direct case management services, including care plans, referrals, and other duties in support of Ryan White as assigned. Adapt training program to changes in volume demands and Social Work and Outreach department growth. Communication Consults with the Human Resources Department staff to support the onboarding, orientation, and competency assessment of all students. Consult with managers about improving staff competence and remediating staff deficits. Collaborates directly with medical providers regarding coordination of care. Proactively develops and maintains positive working relationships and collaborates with medical teams across sites as well as external medical partner entities. Liaison to MD Health Benefit Exchange, HRSA, and connector entities for all Affordable care Act (ACA) outreach and enrollment efforts Planning and Organizing Coordinate with the Department Director professional development offerings of the department, including recruiting speakers, identifying topics of need/interest, and coordinating financial details of offerings. Plans, coordinates, and participates in case conferences with multi-disciplinary teams regarding challenges with specific client issues. Communicates directly with Field Placement liaison regarding student placements from affiliate schools and assists with Field Instructor coordination. Teamwork Work in tandem with the trainee supervisor to provide administrative assistance for new trainees. Assist with aiding team members who are new employees or in need of assistance learning a topic. Monitors and arranges for adequate staffing daily to ensure appropriate Social Work coverage for site. Monitors daily workflow issues and communicates with the Assistant Director, as necessary. Compliance, Policy, and Procedure Document all services as directed by Chase Brexton policy, state requirements and professional standards. Other duties as assigned.
SKILLS AND ABILITIES
Computer literate. Competence in MS Office, Excel, and Word preferred. Excellent verbal and written communication skills. Ability to work with diverse populations. Ability to exercise good judgment and act independently
EDUCATION AND/OR EXPERIENCE
Computer literate Master's Degree in Social Work with active Maryland State Social Work License (LCSW-C) MHBE Insurance Assister/Counselor Certification Four years of related experience in a health care setting and Community, case management, and/or outreach experience At least 1 year of prior experience as a field instructor for MSW students.
