We are a close team of professionals who work well together and maintain a strong focus on the wellbeing of our clients. We are looking for an excited team player able to effectively multitask in a relatively fast paced, ever changing environment. You must be a fast learner and very detail oriented. You must also be available to work on-call every other weekend. No Phone Calls Please. Responsibilities include but are not limited to: Perform duties related to the recruitment, supervision, instruction and motivation of aides. Filling all open shifts with aides based upon the needs of client, employee qualifications, and plan of care. Ability to effectively build one-on-one relationships with both clients and caregivers. Timely and consistently document all information in contact management system. Maintain a current roster of staff, their qualifications/licensures, and their availability. Perform support duties for the nurses in-house and in the field. Monitoring telephony system to ensure caregivers are arriving to client's homes as scheduled. Entering missing times/shifts in a timely fashion. Participate in in-service education, quality assessment activities and other staff meetings as required. Relay pertinent information about patients to the assigned staff, RN and Manager. Maintain an exceptional level of professionalism and responsiveness with prospective clients and others who may communicate with the agency. Receiving and reviewing caregiver care notes.
Minimum job requirements:
High school diploma. Minimum of 1 year experience in personal home care, or related field, as a care coordinator. Direct supervisory experience of at least 1 year. Excellent, professional interpersonal and communication skills - oral, conversational, telephone and written. Effective organizational and task management skills. Ability to demonstrate good judgment. Must work well in fast paced environment and adapt quickly to changing conditions. Must be proactive and able to successfully and effectively adhere to tight deadlines. Must be dependable and able to maintain a steady workflow.Conditions of employment:
