Job Description

Job Summary The Care Management Aide supports the care management team by assisting with care coordination, discharge planning, and administrative tasks. This role provides exposure to interdisciplinary healthcare processes while helping ensure smooth patient transitions and accurate documentation.
Minimum Qualifications Education:
High school diploma or equivalent required Maryland resident Individuals eligible for the Maryland Service Year Program (generally ages 18-24)
Experience:
None required; customer service, volunteer, or healthcare exposure preferred
Licensure/Certification:
None required
