Care Coordinator — Full-Time Help in the Home, LLC | Rockville, MD and surrounding areas | In person Why You'll Love Us Help in the Home, LLC is a privately owned agency that provides personalized care through support and community to help adults with complex mental health needs achieve stability, independence, and inclusion. Position Summary The Care Coordinator develops service plans and coordinates the care of assigned clients. Principal Duties and Required Skills Coordinate client admissions Make ongoing clinical decisions for assigned clients in collaboration with the treatment team and owners Coordinate the medical care of assigned clients Complete client documentation accurately and on time Provide overall care planning and guidance for clients living with severe mental illness Minimum Requirements Master's degree in a mental health-related field with a minimum of two years of related experience, OR a Bachelor's degree in a mental health field with a minimum of five years of mental health-related experience Experience in a psychiatric rehabilitation or clinical care setting working with clients who have severe and persistent mental illness Valid driver's license with a satisfactory driving record; must have access to a personal vehicle and may be required to transport clients Physical Demands Must be able to move efficiently throughout the day, including stairs, and stand and/or sit for extended periods Must be able to lift 10-30 pounds as needed Benefits Health, dental, and vision insurance Health Savings Account Life insurance 401(k) with company match Paid vacation leave Paid holidays Sick and safe leave Wellness leave Flexible schedule
