Job Description:
The Care Coordinator / Community Health Worker plays a vital role in supporting hepatitis C (HCV) outreach, linkage to care, treatment coordination, and follow-up services through a mobile clinic setting. This position works closely with individuals disproportionately impacted by HCV, HIV, sexually transmitted infections (STIs), substance use disorders, homelessness, and co-occurring mental health conditions. This role is responsible for conducting community outreach, building trusting relationships with individuals in underserved communities, supporting engagement in HCV treatment, and helping clients navigate health and human service systems. The Care Coordinator collaborates closely with an interdisciplinary team to provide patient-centered care and improve health outcomes among vulnerable populations.
Key Responsibilities:
Outreach & Community Engagement Conduct targeted outreach to individuals at high risk for HCV, HIV, and STIs, including individuals impacted by homelessness, substance use disorders, or co-occurring mental health conditions Engage in street outreach and mobile clinic activities to build supportive, nonjudgmental relationships with clients Build and maintain rapport with individuals needing linkage to medical care, treatment, and supportive services Provide education regarding HCV prevention, treatment adherence, and available community resources Provide rapid HCV, HIV, and STI testing for clients Assist with mobile clinic operations, including restocking supplies, maintaining organization and cleanliness of the unit, and preparing materials needed for outreach activities Establish and maintain formal and informal partnerships with organizations that have similar or allied goals Represent the organization professionally in community settings and maintain knowledge of organizational programs Care Coordination & Client Support Coordinate linkage to care for individuals who test positive for HCV, HIV, or STIs Support individuals throughout HCV treatment by assisting with follow-up appointments, medication adherence, and ongoing engagement in care Deliver medications to clients as needed Assess client needs and provide referrals to appropriate health and social service resources Collaborate with nurses, peer recovery coaches, case managers, and program leadership to deliver high-quality, person-centered care Document client interactions and services in a timely and accurate manner and assist with program reporting requirements Maintain confidentiality in accordance with
HIPAA, 42 CFR
Part 2, and organizational policies Support all pathways to recovery using a compassionate, trauma-informed, and person-centered approach Attend required meetings and trainings and perform other duties as assigned
Qualifications, Skills & Experience:
Community Health Worker certification required or willingness to obtain certification Must have a valid driver's license, be insurable under the organization's insurance policy, and have access to reliable transportation. Use of a personal vehicle may be required at times as part of job responsibilities. Experience working with individuals affected by substance use disorders, homelessness, mental health conditions, or other vulnerable populations strongly preferred Comfortable working in both mobile outreach and clinic-based environments Strong interpersonal, communication, and relationship-building skills Ability to maintain professional boundaries and work effectively within a team-based environment Knowledge of Microsoft Office and Google Workspace applications Valid driver's license, reliable transportation, and ability to be insured under the organization's policy Ability to pass a background check, sex offender registry check, and drug screening within 48 hours of offer Ability to frequently sit, stand, bend, lift, carry, and reach throughout the workday, including lifting up to 25 pounds Flexibility to occasionally work afternoons and weekends

$24.00 per hour
Work Location:
Services provided throughout Macomb, Oakland, and Wayne Counties.
Schedule:
Typically, scheduled Monday through Friday, with flexibility to work early morning, late afternoon, evening, and occasional weekend hours based on program needs.
