Case Manager (Care Coordinator)

• What You'll Do Conduct in-home assessments with frail and vulnerable seniors Develop individualized care plans tailored to client needs Coordinate essential services and resources Provide ongoing follow-up and monitoring Collaborate with an interdisciplinary team of professionals
• Who We're Looking For A compassionate professional with experience supporting family caregivers and individuals with physical and/or cognitive limitations A Bachelor's degree in Human Services is preferred, but candidates with 2+ years of case management experience are encouraged to apply Must have reliable transportation for home visits within Midland County Basic computer skills are essential
