  • Clinical Hospice Coordinator -
  • Covering referrals for Trinity Health Oakland Hospital-Pontiac
  • Facilitates hospital-to-home transitions by coordinating referrals to hospice services, ensuring timely, effective patient placement.
Serves as a key liaison between hospital staff, physicians, patients, and families—providing education on services, supporting discharge planning, and ensuring a smooth continuum of care. Contributes to relationship development within acute care settings and supports growth initiatives.
  • Minimum Qualifications
  • + Bachelor's degree or equivalent experience required + Graduate of an approved clinical program; current state clinical license preferred + Minimum 2 years of experience in acute care, healthcare sales/marketing, customer service, or related field + Valid driver's license with reliable transportation + Strong verbal and written communication skills + Commitment to organizational mission, ethics, and compliance standards •Key Responsibilities•+ Coordinate and manage hospital referrals, ensuring timely follow-up and service initiation + Assess patient needs, develop care plans, and secure physician orders + Educate healthcare teams, patients, and families on home care/hospice services + Verify insurance coverage and communicate financial responsibilities + Maintain accurate documentation and compliance with all regulations and policies + Build and maintain relationships with hospital staff and referral sources + Participate in quality improvement initiatives and support business development efforts This role requires strong clinical judgment, relationship-building skills, and the ability to manage multiple priorities in a fast-paced healthcare environment.
