Monday-Friday Day Shift Wages:
$30.50 - $35.00/hour DOE Competitive Benefits Package Are you a highly organized clinical professional who thrives on keeping patient care moving smoothly and efficiently? We are seeking a dependable, patient-focused Care Coordinator to play a critical role in supporting our providers, coordinating patient care, and ensuring an exceptional clinic experience. This is a hands-on clinical support role that requires strong follow-through, attention to detail, and the ability to manage multiple moving pieces throughout the patient care process. What You'll Do Room patients and obtain/document vital signs Perform medication reconciliation and update patient records Coordinate clinical follow-up and patient care plans Track patients who have been seen but are not yet scheduled for procedures Assist providers with thyroid biopsies and other office-based procedures Support phlebotomy and basic lab workflows including orders, requisitions, specimen collection, labeling, and tracking Review provider notes and coordinate imaging, labs, and follow-up requirements Communicate with patients regarding next steps, scheduling, and care coordination Help ensure smooth patient flow and operational efficiency throughout the clinic day What Makes Someone Successful in This Role This position is critical to running the clinic efficiently and safely. The most important characteristic for success is strong organization and reliable follow-through on patient tracking and care coordination. We are looking for someone who is proactive, detail-oriented, and committed to ensuring that no patient "falls through the cracks." Qualifications Graduate of a Medical Assistant or Practical Nursing program CMA certification eligibility or LPN licensure within 3 months BLS/CPR certified or willing to obtain certification Strong communication and interpersonal skills Ability to work effectively in a fast-paced clinical environment Minimum 1 year of clinical experience preferred Why Join Us?
