Minneapolis (Elliot Park)
Pay Range:
$20.00 to $25.00 an hour
Schedule:
Full-Time, 40 hours per week. Monday
  • Tuesday 8:00am
  • 4:30pm, Wednesday 2:00pm
  • 10:00pm, Thursday 10:00am
  • 4:30pm, Friday 8:00am
  • 4:30pm.
Community Integration Coordinator Responsibilities:
Establish supportive and motivational relationships with residents; coach residents to utilize coping strategies and complete activities of daily living; Observe residents for changes in health symptoms; redirect maladaptive behaviors; perform behavioral management interventions; and de-escalate mental health crises. Complete intake process for new residents and complete discharge process for residents exiting the program. Collaborate with residents and their care teams to develop goals, complete functional assessments, and complete individual abuse prevention plans. Meet daily with clients to support client goals and needs for managing their illness; and encourage the use of natural supports and community resources. Assist in the completion of independent living skills, including meal planning, budgeting, shopping, housekeeping, and laundry. Refer residents to appropriate services and community resources; and assist with scheduling appointments and transportation. Perform delegated nursing services, including the administration of medications, as unlicensed personnel under the direction of the nursing staff. Ensure residents and their guests are following site guidelines to ensure the facility is safe and secure. Accompany residents on appointments as needed. Maintain client records in an electronic health record system. Maintain communication with providers, family members and others to promote the well-being of residents. Collaborate with other staff members, consultants, interns, and/or volunteers.
Community Integration Coordinator Requirements:
1-2 years of experience, post secondary education in behavioral health, or combination of experience and education required Valid driver's license and acceptable driving record required Experience working with adults with SPMI, chemical dependency issues, and co-occurring disorders preferred
