This position offers the right candidate a challenging position with an opportunity for growth. Candidate must be highly organized, detail-oriented, able to multi-task, & think on their feet in a fast paced, demanding, and rewarding position. Performs day-to-day coordination of care delivered by aide/nurse staff in the homes of Phoenix clients. Develops and maintains ongoing relationships with clients and family in order to best meet the needs of the client. Represents the department and company in a professional and courteous manner at all times.

Job Duties:
Represents the department in a courteous and professional manner. Schedules open visits with field staff based upon the needs of client, employee qualifications and plan of care or treatment. Contacts "open" clients within thirty (30) minutes of any pertinent changes in their care. Performs support duties related to the supervision, instruction and motivation of aide/nurse staff and include related documentation of such activities. Respond to client/employee problems as requested by Supervisor/Manager Document and inform scheduling supervisor of scheduling changes, hotlines, performance/ attendance issues and any other problems occurring in the field. Prepares and submits timely written documentation and reports as required by policy. Assists with division reports as instructed by supervisor. Performs clerical functions such as filing, copying, distributing, logging and mail sorting. Participates in in-service education, quality assessment activities and staff meetings as required. Provides client care as necessary; performs visits to evaluate in-home aide job performance, including documentation of observances. Assist with information gathering and problem resolution when client or employee concern/ complaints are presented. Performs other related duties as assigned by Supervisor/Manager.
Requirements:
Must be a minimum of 18 years of age High school diploma or equivalent Supervisory Experience preferred 1 year experience working with the elderly, disabled or infirmed Office Setting Experience One Year minimum experience working with elderly and/or disabled Ability to work independently with excellent organizational skills Ability to communicate effectively and courteously with consumers and perform in-depth interviews with consumers. Must work well in a fast-paced environment and adapt quickly to changing conditions.
