Role:

Coordinate and complete MDS assessments in accordance with federal and state regulations Develop, implement, and update individualized care plans Collaborate with interdisciplinary team members to ensure accurate documentation and quality outcomes Monitor compliance with CMS guidelines and support survey readiness Participate in care plan meetings with residents and families
