JOB SUMMARY
Direct Care Coordinators coordinate and oversee schedules of caregivers and clients enrolled in Medicaid programs.
JOB DUTIES
Schedule caregivers to meet clients' in-home care needs in compliance with regulations and Managed Care Organization ( MCO ) program rulesAssist with daily intake and referral processes to ensure positive business growth and developmentPromote cohesiveness in the office environmentSupport day-to-day operations and client advocacyCollaborate with stakeholders regarding the delivery of servicesCommunicate effectivelyComply with applicable legal requirements, standards, policies, and proceduresDemonstrate dependabilityDemonstrate effective problem solving and decision-making skillsExhibit computer efficiencyHandle complex service programs and special projectsMaintain necessary skills and knowledge to coordinate workflowMarket services to referral sourcesMonitor deliverables and ensure timely completion of projectsParticipate in professional development and training activitiesPrioritize and multitask effectivelyProvide excellent customer service to internal and external clientsRepresent the company at stakeholder meetings, health fairs, and provider fairs as neededStaff new client referrals and open client shifts with available agency caregiversAssist in hiring and recruiting new caregivers and processing new caregiver paperworkProvide backup personal care services to Medicaid members in their homes as neededBilingual preferred in various locationsOther duties as assigned
QUALIFICATIONS
Associate's Degree preferredPrevious experience serving individuals with disabilities preferredA combination of education and experienceThe incumbent typically works in an office environment and uses a computer, telephone, and other office equipment as needed to perform duties. The noise level in the work environment is typical of that of an office. Incumbent may encounter frequent interruptions throughout the workday. The employee is regularly required to sit, talk, or hear; frequently required to use repetitive hand motion, handle or feel, and to stand, walk, reach, bend or lift up to 20 pounds.
