Position Summary The Resident Care Coordinator (RCC) is responsible for overseeing the day-to-day care operations of the facility, ensuring residents receive consistent, high-quality care while supervising staff and coordinating healthcare services. This role serves as the central point of communication between staff, residents, families, and healthcare providers, while supporting the Administrator in maintaining smooth operations and regulatory compliance. Staff Supervision & Leadership Supervise direct care staff (CNAs, Med Techs, aides) Provide daily guidance, direction, and support Address call-outs, shift coverage, and scheduling issues Monitor staff performance and report concerns Assist with training and onboarding Scheduling & Staffing Create and manage staff schedules Ensure proper coverage for all shifts Handle last-minute call-outs Maintain compliance with staffing requirements Resident Care Oversight Ensure care is delivered according to care plans Monitor resident conditions and report changes Communicate with families regarding care needs Support medication administration oversight Healthcare Coordination & Provider Management Coordinate and oversee provider visits (physicians, nurse practitioners, hospice, therapy, etc.) Communicate resident needs and condition changes to providers Receive and implement new orders and medication changes Ensure medication updates are properly communicated and documented Schedule outside medical appointments Coordinate transportation for appointments Track and follow up on provider recommendations Daily Operations Manage day-to-day floor operations and problem-solving Respond to staff and resident needs in real time Ensure coordination between departments Compliance & Documentation Ensure staff follow policies and procedures Assist with documentation and regulatory compliance Support state survey preparation Ensure incident reports are completed accurately Communication Serve as liaison between staff and Administrator Communicate updates clearly to team members Participate in leadership meetings Qualifications Experience in assisted living, long-term care, or healthcare Supervisory experience preferred Strong organizational and problem-solving skills Ability to multitask in a fast-paced environment Strong communication skills Familiarity with medical terminology preferred Full-time position Some evenings/weekends as needed On-call rotation may be required
