share to e-mail Tweet share to twitter Share on Facebook share to facebook Share on LinkedIn share to linkedin Current Team Member - Apply Now Apply Now Save Job Job Saved Job Description Position Summary As the Trauma Resiliency Care Coordinator, you will be central to the Trauma Services department's commitment to providing holistic, patient-centered care that extends beyond physical injury to address the complex psychosocial needs of trauma survivors and their families. In this role, you will implement and sustain trauma recovery services through the Trauma Survivors Network (TSN), working directly with participants and consistently applying traumainformed, evidencebased practices. You will also help build a community of support for patients and families as they navigate recovery by coordinating peer visitation programs, support groups, and educational classes. Through this integrated approach, needs are identified early and patients are connected to the right resources, fostering optimal outcomes and longterm resiliency. You will conduct universal screenings for trauma patients and families to identify risks for Post-Traumatic Stress Disorder (PTSD), depression, and substance misuse. The Trauma Resiliency Care Coordinator is accountable for a designated patient caseload and plans effectively in order to meet patient needs, promote efficient utilization of resources and ensure a well-organized and safe plan of care is established for every patient. Minimum Requirements Master's of Science in Social Work or higher is required Licensed Clinical Social Worker -(LCSW) is required. Experience in trauma, critical care, emergency medicine, or medical case management is strongly preferred. 2 years of experience is preferred.An advanced certification also recommended:
Accredited Case Manager (ACM) or Certified Care Manager (CCM), Certified Advanced Social Work Case Manager (C-ASWCM), Medical Case Management Clinical Coach Certification, or Health Coach.Pay Range Pay Range:
