As a State Certified Adult Day Care/Adult Day Health Center, ESEC is dedicated to providing compassionate, community-based care for frail and impaired adults requiring daytime supervision, medical screening, and social engagement.
Job Summary:
We are seeking a dynamic Healthcare Coordinator to join our dedicated team. In this vital role, you will coordinate comprehensive healthcare services for our adult participants, ensuring seamless communication between medical providers, families, and staff. Your expertise will help promote optimal health outcomes, manage complex care plans, and support the overall well-being of our clients in a nurturing environment.
Duties:
Conduct health assessments and develop individualized care plans. Collaborate with healthcare providers, families, and caregivers. Regularly monitor participants' health and document changes. Communicate health updates to relevant parties. Provide education on health conditions and management to participants and staff. Offer emotional support and resources to participants and families. Ensure compliance with healthcare regulation and program policies. Maintain accurate records of care and services provided.
Qualifications:
Education:
Degree in nursing (LPN or RN)
Experience:
Previous experience in healthcare coordination preferred.
Certifications:
CPR and First Aid certifications required.
Experience:
Previous experience writing care plans, caregiving, healthcare.
Pay:
$23.08 - $27.79 per hour
