Job Description

Job Overview:
We are seeking an experienced Resident Care Coordinator to join our team. In this role, you will oversee and coordinate resident care, ensuring their safety, well-being, and overall quality of life. The ideal candidate will possess strong organizational and communication skills, along with a passion for providing hight-quality care.
Responsibilities:
Coordinate and schedule resident appointments, ensuring all necessary services are provided Conduct assessments to determine resident needs and develop individualized care plans Monitor resident progress and make adjustments to care plans as needed Collaborate with healthcare professionals, caregivers, and family members to ensure coordinated care Provide guidance and support to caregivers, offering training and resources as needed Assist with medication management and administration Monitor vital signs and report any changes or concerns to healthcare providers Implement behavior management strategies for clients with challenging behaviors Maintain accurate and up-to-date client records
Qualifications:
First aid certification required Experience in caregiving or senior care preferred Knowledge of assisted living facilities and home care services Familiarity with social work principles and practices Strong organizational skills with the ability to multitask effectively Excellent communication and interpersonal skills Ability to lift and assist clients as needed Valid driver's license with a clean driving record Our goal is to create positive experiences for our residents and their families. Applicants must be authorized to work in the United States and must successfully complete all required pre-employment screenings in accordance with company and state guidelines.

$16.00 - $18.00 per hour
