Better Together ND provides care coordination, peer support, and recovery services aligned to each individual's specific needs. Better Together ND is hiring for Care Coordinators in Fargo, ND.

Position Summary:
Care Coordinators empower individuals to reach and maintain their highest level of physical and mental well-being. As a care coordinator you will have regular meetings with individuals involved in our programs. Care Coordinators will assess the client's needs and develop a participant-centered care plan and client-driven goals. Care Coordinators will work together with local agencies and care teams to ensure success in the realms of recovery, housing, employment, and recidivism. Care Coordinators are expected to meet with participants once a week and document each interaction in a timely manner.
Required qualifications:
Bachelor's degree in a closely related field to the positions responsibility or have four (4) years of experience in a human service setting providing direct services to individuals. Care Coordinators are required to complete the care coordination training provided by the State of North Dakota after being hired. Valid Driver's License Must successfully complete a background check. Preferred Qualifications Case management experience preferred but not required Excellent writing skills Self-starter Pay $4,000 per month Hours Full-time Flexible work hours - Care Coordinators schedule their appointments which gives them the freedom to work when they want.
