DEPARTMENT FUNCTION/JOB OVERVIEW
Responsible for using clinical data and various assessment tools to plan, coordinate and evaluate results of interventions to provide rehabilitative patient care. Assesses and re-assesses patient condition and progress towards goals, reports changes to physicians/providers, appropriate team members and family. Utilizes critical judgment for day to day shift assignments and managing resources. Work will be performed in an ethical and legal manner following organizational policies, processes, protocols and procedures. Responsible for quality service delivery and internal/external customer relations for Madonna as a whole, including upholding the mission and values for the department and facility.
ESSENTIAL FUNCTIONS
(INFORMATION) Complete and analyze patient care assignments based on patient census, acuity, needs, staff qualifications and competencies and to promote continuity of care. Compute, analyze, and coordinate personnel information to optimize delivery of patient care during designated shift. Implement physician orders to provide holistic approach to patient care. Coordinate, compile, analyze and record patient information to establish and evaluate patient's progress toward goals and executing the nursing plan of care. Analyze and coordinate research data to contribute current information to patient's plan of care. Coordinate program and facility policies, procedures and job descriptions to ensure quality patient care delivery. Document personnel, patient, family, and visitor issues, interventions, and plans of correction. Collect quality assurance data, analyze data, implement improvement plans as assigned by supervisor Collect data for internal research projects, coordinate research activities as assigned by supervisor. (PEOPLE) Supervise, mentor, and instruct departmental staff, students, and new employees to ensure quality care, as well as compliance with standards. Develop rapport with staff, patients, families, and other health care providers to arrive at decisions for the benefit of patients in an interdisciplinary setting through goal setting, progress reporting and being a patient advocate. Mentor staff to provide quality patient care on a daily basis, as well as through meetings and individual performance appraisals. Demonstrate a high level of clinical expertise, mentor and educate to ensure continuous clinical improvement. Communicate with patients and families by providing clinical updates, progress towards goals and responding to their physical and emotional needs. . Assisting with patient care on a day to day basis. Provide education to patient and families by explanation, demonstration and supervised practice to enable patients to be as independent as possible. Follow instruction from supervisor to perform other functions as assigned in order to achieve the goals within the department and clinical services system. Collaborate with all departments to obtain necessary support services for patients. Assist staff in evaluation of patients to ensure that proper care and ancillary departments are utilized for patient outcome. These departments may be but not limited to pharmacy, radiology, therapies, lab, etc. Collaborate with the administrator on call for situations that place the facility at risk. Assist employee health nurse with employee health issues, triage injuries and implement interventions. Reference for weather warnings, disaster, and code responses. (THINGS) Maintain a clean, organized and safe environment. Handle and operate all necessary equipment and perform required duties according to established safety standards to maintain compliance with regulations and prevent injury. Coordinate procurement of essential patient care equipment and supplies when purchasing and central services are closed. Be able to operate personal computer for clinical documentation and review, word processing, e-mail, policies, procedures, and education training and testing. Medical equipment used on patient care units may include but is not limited to: stethoscope/sphygmomanometer blood glucose monitor thermometer feeding pump suction O2 ventilator telephone paging system IV infusion pump beds, wheelchairs, stretchers trauma tub digital wheelchair scale and bed scale safe patient handling equipment such as ceiling lift, standing lift, mechanical lift, portable and stationary vital sign monitor WOWs bladder scanner
PHYSICAL DEMANDS AND ENVIRONMENTAL CONDITIONS
Medium work with frequent lifting. Manual patient transfer or reposition which does not exceed the equivalent of 35 pounds of force to lift, push or pull. Requires sufficient strength to produce 50 pounds of force to push a wheelchair with a 300-pound individual on level surfaces. Constant walking with maximum duration of up 30 minutes sustained walking on tile, carpet and other surfaces. Must tolerate frequent standing with sustained periods of up to 1 hour on tile, carpet, and shower surfaces. Must tolerate frequent sitting with sustained periods of up to 1 hour. Frequent reaching with both arms at low, waist, and high levels. Frequent grasping of both hands for transfers and repositioning requiring normal grip strength based on national averages. Frequent stooping, bending, and twisting are required for patient care. Constant near visual acuity to evaluate and monitor patients. Ordinary hearing and speaking skills. May be exposed to biohazardous materials.
QUALIFICATIONS
(Education/training and/or Experience) Must possess current Nebraska R.N. licensure. Requires current CPR for Health Care Provider/CPR for Professional Rescuer certification, two years' experience in hospital, geriatrics or community health nursing required or demonstrated leadership skills within a position at Madonna Rehabilitation Hospital. Background checking is conducted on applicants for this position with the agencies as required by Nebraska State regulations, in addition to any other agencies as determined by Madonna. Applicants must sign authorization forms (when required by the agency) to release information to Madonna.
