As our Social Services Coordinator, you will serve as a trusted advocate, guiding care planning, discharge transitions, and emotional support with compassion and professionalism. Position Summary The Social Services Coordinator plays a vital role in supporting the emotional, psychosocial, and transitional needs of residents within our skilled nursing and short-stay rehabilitation community. This individual serves as an advocate for residents and families, ensuring a positive experience from admission through discharge, while collaborating closely with the interdisciplinary team to promote high-quality outcomes and resident satisfaction. Key Responsibilities Conduct comprehensive psychosocial assessments upon admission and throughout the resident's stay Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team Provide counseling, emotional support, and crisis intervention to residents and families Assist with discharge planning to ensure smooth, safe transitions to home or other levels of care Coordinate with community resources and external agencies to support resident needs Facilitate care conferences and communicate updates to residents and families Address resident concerns, grievances, and quality-of-life needs in a timely and professional manner Support advance care planning discussions as appropriate Maintain accurate, timely documentation in accordance with state and federal regulations Promote resident rights, dignity, and person-centered care Qualifications Bachelor's degree in Social Work, Human Services, or related field required (BSW preferred) Experience in skilled nursing, rehabilitation, or healthcare setting preferred Strong knowledge of discharge planning processes and community resources Excellent interpersonal, communication, and conflict resolution skills Ability to manage multiple priorities in a fast-paced healthcare environment Knowledge of state and federal long-term care regulations What We Offer Competitive benefits package including low premiums, short-term disability, long-term disability, HSA contribution matching, 401(k) with company match, and more!
