Care Coordinator 3.1 3.1 out of 5 stars
Summary:
The Care Coordinator serves as the bridge between operational staffing and peer engagement. This position is responsible for staffing peers with Certified Peer Specialists while ensuring services are aligned with Individual Recovery Plans and fully utilized. We are seeking a candidate with home care staffing experience who understands how to balance coverage management with person centered service delivery.

Staff peers with Certified Peer Specialists based on IRP authorization, availability, and geographic needs Monitor utilization to ensure authorized hours are maximized Identify and resolve service gaps or underutilization trends Conduct outreach to support peer engagement and retention Collaborate with CPS and leadership to maintain consistent service delivery Track scheduling trends and recommend operational improvements Maintain compliant documentation and follow HIPAA standards Qualifications 2 years of experience in staffing, scheduling, or care coordination Previous home care staffing experience preferred Experience in behavioral health or Medicaid programs a plus Strong communication and organizational skills Comfortable working in a fast paced, in person office environment
