Care Coordinator Requirements:

Proficient in Microsoft Office applications 1 year of experience in case management marketing 2 years of experience in case management marketing preferred One of the following licenses or a bachelor's degree in a related field from an accredited college or university: LPN RN LCSW

What You'll Do:

Gather and evaluate referral information, review submitted materials, and/or assess potential residents for admission Make admission decisions and/or collaborate with nursing staff on healthcare admission determinations Build and maintain a referral network, including conducting onsite visits as needed Facilitate the admission process into healthcare, ensuring all required paperwork is completed and processed in a timely manner Verify Medicare coverage and/or other insurance benefits Participate in care conferences and other required meetings Follow up with residents post-discharge to ensure well-being; complete medication reconciliation and psychosocial assessments Support post-acute discharge care in the home setting Ensure departmental reports are completed accurately and on time Why Work at UMC (United Methodist Communities)?
