As a Home Care Coordinator, you will play a vital role in connecting patients with the care they need, while also fostering meaningful relationships with our clients and their loved ones. Responsibilities:
Coordinate and manage the care plans of patients, ensuring that their needs are met and exceeded Conduct patient assessments and develop individualized care plans in collaboration with healthcare professionals Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions and continuity of care Identify and address potential issues or concerns, taking proactive steps to resolve them Maintain accurate and up-to-date records of patient care and services provided Participate in multidisciplinary team meetings to share knowledge and expertise Collaborate with other departments to ensure efficient and effective care delivery Scheduler Coordinator when needed
