We provide comprehensive, evidence-based care across a full continuum of services tailored to meet the unique clinical needs of each individual, empowering them to achieve lasting sobriety and holistic well-being. Our programming spans the full continuum of care, including inpatient medical detoxification, residential programming, Intensive Outpatient Programming (IOP), sober living, aftercare services, and individual therapy. Complementing our clinical programming is an extensive Certified Peer Support Worker (CPSW) program, through which individuals with lived experience in recovery provide direct support, engagement, and connection throughout a client's time with us. POSITION OVERVIEW
The Clinical Care Coordinator is a licensed clinician who serves as a primary clinical point of contact for individuals seeking treatment at Turning Point Recovery Center. This is a specialized, time-limited direct service role focused on comprehensive clinical assessment, diagnostic formulation, level of care placement, crisis intervention support, and the identification of targeted case management needs to support a client's entry into programming. The Clinical Care Coordinator facilitates the bridge between a client's first contact with our organization and their entry into the appropriate level of care. This position carries significant clinical responsibility. Joining a seasoned clinical team, the Clinical Care Coordinator assists in conducting diagnostic interviews, completing comprehensive biopsychosocial assessments, applying ASAM criteria to determine the appropriate level of care, and initiating treatment planning upon admission. Because this clinician is often among the first to sit across from a client at one of the most vulnerable and decisive moments of their life, clinical competence must be matched with genuine warmth, cultural humility, and a trauma-informed lens that centers safety, trust, and empowerment from the very first interaction. The Clinical Care Coordinator must also be prepared to respond to clients presenting in an active crisis. This requires proficiency in crisis assessment, safety planning, and de-escalation, as well as the clinical judgment to determine when immediate coordination with medical or psychiatric staff is necessary. The Clinical Care Coordinator is an integral part of the multidisciplinary team, collaborating closely with the Clinical Director, Executive Director, medical providers, and therapists to ensure appropriate placements and that client needs are identified and addressed from the moment of entry. This individual participates in and presents cases at treatment team meetings and serves as a key communication link across the continuum of care.
ESSENTIAL DUTIES
Clinical Assessment and Screening Conduct structured diagnostic interviews and comprehensive biopsychosocial assessments for all incoming clients. Administer and interpret clinical screening tools, including the Columbia Suicide Severity Rating Scale (C-SSRS), Beck Depression Inventory, Burns Anxiety Inventory, and other validated instruments as clinically indicated. Apply foundational knowledge of the DSM-5 to identify and document diagnostic impressions for substance use disorders and co-occurring mental health conditions. Gather relevant history including trauma, psychiatric, medical, legal, and social history to inform a thorough clinical formulation. Conduct safety assessments, develop safety plans when indicated, and coordinate with the medical team to ensure client safety during the intake process. Level of Care Determination and Placement Apply ASAM 3rd Edition criteria (required) and ASAM 4th Edition criteria (preferred) to determine the clinically appropriate level of care for each client. Demonstrate strong knowledge of exclusionary criteria and use sound clinical judgment to assess appropriateness for programming based on severity of symptoms, diagnoses, and clinical presentation. Collaborate with the Clinical Director, Executive Director, psychiatric provider, medical team, and therapists to confirm placement decisions and facilitate timely admission into the appropriate level of care. Communicate level of care determinations and clinical rationale clearly to the treatment team and to referral sources. Initial Treatment Planning Develop individualized initial treatment plans with clients upon entry into programming, identifying presenting problems, measurable goals, and clinical objectives. Integrate client strengths, needs, abilities, and preferences into treatment planning using a strengths-based, person-centered approach. Ensure treatment plans reflect ASAM-aligned clinical priorities and are consistent with the client's identified level of care. Brief and Targeted Case Management Conduct time-limited case management activities focused on facilitating entry into treatment, identifying immediate barriers, and connecting clients to appropriate internal and external resources. Coordinate with the peer support team, medical staff, and clinical team to address immediate client needs during the intake and admission period. Present client cases and clinical findings at multidisciplinary treatment team meetings, contributing to collaborative care decisions. Care Coordination and Communication Coordinate client care across the clinical team, including the Clinical Director, Executive Director, psychiatric provider, primary therapists, financial, and peer support specialists. Serve as a key point of contact and communication link during the intake and admission process, ensuring seamless transitions into programming. Communicate effectively with referral sources, including courts, probation and parole, the Children, Youth and Families Department (CYFD), hospitals, military branches, the Board of Nursing, aviation and flight programs, and self-referrals. Maintain collaborative relationships with external providers and community partners to support continuity of care. Documentation and Compliance Complete all clinical documentation with a high degree of accuracy, clarity, and professionalism, including diagnostic impressions, biopsychosocial assessments, safety plans, and initial treatment plans. Adhere to a 24-hour documentation turnaround expectation in compliance with insurance requirements and stakeholder demands. Maintain accurate and up-to-date records in the Electronic Health Records (EHR) system in accordance with agency policies, regulatory requirements, and ethical standards. Crisis Support Provide crisis intervention and stabilization support during the intake process, working closely with the medical and clinical team to ensure client safety. Demonstrate proficiency in safety screening tools, safety planning frameworks, and de-escalation techniques. Coordinate with clinical and medical staff when a client's presentation requires immediate or elevated intervention beyond the scope of the intake process. Professional Development Stay current with developments in the field of substance use disorder treatment, co-occurring mental health, and clinical assessment practices. Participate in training, supervision, and professional development activities as required.
QUALIFICATIONS

Licensed Master Social Worker (LMSW) Licensed Clinical Social Worker (LCSW) Licensed Mental Health Counselor (LMHC) Licensed Marriage and Family Therapist (LMFT) Licensed Professional Clinical Counselor (LPCC) Education and Experience Master's degree in social work, Counseling, Psychology, or a related behavioral health field. Minimum of 2 years of direct clinical care and counseling experience in substance use, behavioral health, or a related setting. Demonstrated experience conducting clinical assessments, diagnostic interviews, and case conceptualization. Strong foundational knowledge of ASAM 3rd Edition criteria required; familiarity with ASAM 4th Edition preferred. Working knowledge of the DSM-5 and diagnostic criteria for substance use and co-occurring mental health disorders. Experience with safety screening tools, safety planning, and crisis intervention. Experience communicating with diverse referral sources, including legal systems, child welfare agencies, healthcare institutions, and regulatory bodies, preferred.

Advanced clinical assessment skills, including diagnostic interviewing, case conceptualization, and clinical formulation Demonstrated knowledge of ASAM criteria and its application to level of care determination across residential, outpatient, and inpatient settings Strong working knowledge of substance use disorder treatment modalities, recovery principles, and co-occurring mental health disorders Ability to build rapport and trust quickly with individuals in acute distress or ambivalence about treatment Proficiency in writing clear, concise, and clinically precise documentation, including assessments, treatment plans, and progress notes Proficient in Electronic Health Records systems Proficiency in Microsoft Office Suite and Google Workspace Excellent interpersonal and communication skills across multidisciplinary teams and external referral sources Ability to work independently, prioritize competing demands, and pivot effectively in a fast-paced clinical environment Strong organizational skills and rigorous attention to detail, particularly as it relates to documentation accuracy and timeliness Commitment to ethical practice, client confidentiality, and professional standards of conduct Valid driver's license and reliable transportation
PHYSICAL REQUIREMENTS
Ability to sit, stand, and walk for extended periods as required by the position. Use of a computer, keyboard, and monitor is required. Must be able to lift and carry up to 15 pounds. Must be able to communicate verbally and in writing in English; bilingual proficiency in English and Spanish is a plus and reflective of the diverse communities we serve. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
