General Service Provider Visions Case Management & Consulting Albuquerque, NM Job Details $55,000 a year 16 hours ago Benefits Health insurance Dental insurance 401(k) Flexible schedule Qualifications Computer operation Customer communication Computer literacy Math Bachelor's degree Technical Proficiency Full Job Description As a Service Provider at Visions Case Management & Consulting, you will be strengthening the communities of New Mexico by enhancing the lives of those we serve and work with. Will also be a source of information, to foster a supportive environment, to deliver exceptional services, and to empower everyone you interact with.
Responsibilities:
You will have the following responsibilities: Facilitate monthly contact with consumers and submit related documentation in a timely manner. Drafting and submitting annual plans/budgets according to program timelines and revise as needed. Maintain complete and current client case file. Utilize and update internal tracking systems 2 x weekly and ensure information is current and complete. Specifically google sheets or spreadsheets in general Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training. Respond to internal and external communications within 24 hours.
Required Qualifications:
Four Year Degree in Nursing, Psychology, Social Work and 1 year of related experience Any Four Year Degree and 6 years of related experience Travel within an entire designated region of the state
