PFY is seeking to hire a full-time Care Coordinator who will have the primary role of providing strengths-based case management services aimed at supporting engagement in sexual health and essential support services . To be successful in their role, the Care Coordinator, will have an understanding of the needs of LGBTQ+ young people of color and demonstrate the core values of the agency; respect, integrity, inclusiveness, collaboration, and dependability. Responsibilities:
1) Provide community and social media-based outreach to young gay and bisexual men and transgender individuals (ages 13-30) throughout Suffolk County, Long Island, with a focus on African American and Latino populations for PEP/PrEP services, HIV and rapid syphilis testing. Community outreach venues will include local colleges, bars, nightclubs, street/neighborhood locations etc.. Social media outreach will include Facebook, Twitter, Tumblr and dating apps such as Grindr and Jack'd. Outreach will include tabling, giving out safer sex packets, providing education on PEP/PrEP and HIV testing services. 2) Provide in-person and virtual confidential HIV, confirmatory HIV testing, and rapid syphilis testing and counseling for high-risk individuals during agency programming, via mobile testing van, and by appointment, with a particular focus on young gay and bisexual men and transgender women of color. 3) Conduct PrEP/PEP evaluations with high-risk individuals with a particular focus on gay/bi/queer men and transgender women. This includes coordinating linkage into medical services with ongoing monitoring for all PrEP/PEP enrolled clients. 4) Provide ongoing PrEP/PEP support services which may include client escorting, insurance navigation, care coordination, biomedical screenings, appointment reminders, and adherence/retention counseling. 5) Carry a caseload of (25-35) case management HIV Navigation Services (HNS) clients. This includes but is not limited to developing client centered action plans, service/provider coordination, client follow ups, file organization, and data service tracking. 6) Provide evaluation/linkages to medical services and ongoing monitoring for all PEP/PrEP clients. 7) Coordinate Project Push program which includes but is not limited to; client outreach/engagement efforts, insurance verification, lab follow ups and client service linkages 8) Review and develop targeted outreach plans with Prevention Services Manager on a quarterly basis. 9) Adhere to all OSHA, HIPAA, and
HIV/AIDS
Confidentiality laws and regulations. 10) Maintain all required electronic and written records of testing, linkages and outreach activities, in addition to maintaining monthly statistics of clients served. 11) Responsible for monthly stats and narrative submissions to reflect work conducted. 12) Additional activities as assigned by the
