Job Description

Job Summary:
This position is a full-time position consisting of 40 hours per week. The ER Coordinator will be responsible for all discharge planning activities for patients in collaboration with the patient's treatment team. Schedule may vary depending on usage in the emergency department.
Qualifications:
Bachelor's Degree with 2 years Care Coordination/Management experiences; or associate degree with 4+ years Care Coordination/Management experience. Reliable, trustworthy, sound decision making skills. Strong, verbal, writing and organizational skills. Strong computer skills. Demonstrated ability to assist interagency and outside agency employees in a professional, welcoming, and efficient manner. Ability to maintain confidentiality of patients in the emergency room. Full comprehension of HIPPA regulations. Availability to maintain a flexible work schedule. Ability to pass NYS Criminal Justice Background check, Office of Family and Children Service Background and not be listed on the NYS Staff Exclusion list, OIG, and HHS-OIG. Must complete all required trainings for this position.
Job Responsibility:
Must complete Mandated Reporting and HIPAA training within the first week of employment. Must obtain NYS Peer Certification within first 6 months of employment. Must obtain an additional 10 hours of CEUs annually after receipt of New York State Peer Specialist Certification to maintain credentials. Keep statistical data to report to ACO by the last working day of the month. Must engage 80% of high utilizers Must have knowledge of resources and systematic processes in Franklin County. Must abide by all agency policies and procedures. Must adhered to professional boundaries with fellow employees, individuals receiving service and partner's employees. Must maintain the confidentiality of individuals you work with. Attend required meetings and or trainings requested by the agency and or partner agency. Engage in supervision meetings with your direct supervisor. Help to connect individuals to providers they deem suitable for their overall well-being such as, but not limited to, primary care, etc. Connect by making appointments, scheduling MAS transport and more. Ability to focus on the root cause of issues and not just the issue presented. Complete continued education on recovery to help individuals with whom you are working. Maintain a professional working relationship with all agencies on behalf of CCFC. Must always deliver a person-centered service model. Review social determinants that may be affecting an individual or family's overall health. Empower individuals you are working with by offering information and education. Connect with Health Home Care Managers quarterly to discuss individuals working with. Determine patients discharge plan in collaboration with the patients assigned treatment team. Coordinate discharge planning activities by making all necessary authorizations from insurance companies/funding sources as determined by the treatment plan. Document all care coordination activities. Build professional working relationships with clinic and emergency room staff that will further benefit the patient's outcomes. Working in collaboration with patient, and assigned treatment team, uncover barriers to care and develop an effective plan to alleviate these barriers to care for the safety of the patient. Collaborate with treatment team to determine high risk medical/social management patients. Serve as a liaison with other service providers or community supports for continuous improvement of services for patients. Adhere to all CCFC policies and procedures. Adhere to UVM Alice Hyde Medical Centers and or Adirondack Health's policies and procedures. Other duties assigned by direct supervisor.
