Job Description

Compensation Range:
$60,000-$65,000 commensurate with experience.
Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.
Overview:
The Housing Works Health Home Care Coordination Program partners with medical and behavioral health providers to deliver coordinated, high-quality care for Medicaid recipients with complex health needs or those at risk of high service utilization. Our work focuses on improving access to care, reducing barriers, and supporting meaningful long-term health outcomes. As a Care Coordination Supervisor, you will play a key leadership role guiding Care Managers and Care Navigators as they engage clients, complete comprehensive assessments, and develop individualized, goal‑driven care plans. You will help build staff capacity, strengthen clinical and engagement skills, and ensure high-quality, person-centered service delivery. Supervisors utilize a dynamic, data-informed supervision model, including clinical supervision, coaching, training, and performance feedback. You will collaborate with interdisciplinary healthcare partners and contribute to ongoing quality improvement initiatives.
Essential Job Functions:
Provide clinical and administrative supervision to care managers to support the development of effective interventions and treatment plans. Maintain a consistent in-office presence during core business hours to support walk‑in clients and provide real-time staff supervision and crisis support. Facilitate and participate in care conferences and in‑person engagements to coordinate services effectively. Lead in-person team huddles and case conferencing sessions to foster collaboration and ensure alignment on care strategies. Conduct in-office onboarding and shadowing for new staff to ensure familiarity with agency protocols, engagement standards, and documentation practices. Perform regular in-person observations of care manager-client interactions to assess service quality and ensure compliance with Health Home standards. Serve as an in-office anchor for interdisciplinary coordination with medical, behavioral health, and social service partners. Routinely update staff on program and agency policies and procedures. Participate in internal and external trainings to enhance program practice and improve client outcomes. Monitor delivery of services for compliance with program requirements. Provide crisis intervention for clients experiencing psychological distress. Participate in staff meetings to review program and agency priorities. Engage staff in meetings and small groups to gather input, feedback, and suggestions. Review and approve client documentation; provide feedback to care management staff as needed. Utilize reports to ensure billing targets and performance metrics are achieved. Consistently demonstrate Housing Works core values: All In, Stronger Together, High Performance, and Membership. Participate in quality assurance and quality improvement activities. Conduct field visits to support client engagement, assess service delivery, and meet Health Home program requirements. Engage in advocacy efforts to help end homelessness and AIDS. Perform other duties as assigned.
Education Requirements:
Master's Degree required: Social Work (LMSW), Nursing (RN), Psychology, Public Health (MPH), Human Services, or related field. Minimum of two (2) years of supervisory and administrative experience.
Qualifications & Skills:
Proficiency with Microsoft Office (Word, Excel); Electronic Health Records experience is a plus. Strong written and verbal communication skills. Excellent organizational, multitasking, and time-management abilities. Strong analytical skills for collecting, organizing, and interpreting information. Positive, adaptable attitude when working with both staff and clients. Highly organized with the ability to work independently in a fast-paced environment. Comfortable working with diverse populations.
Competencies:
Proficiency with Zoom, Microsoft Teams, and other virtual meeting platforms. Understanding of chronic medical conditions, homelessness, substance use, and mental illness. Knowledge of City, State, and Federal entitlement and benefit systems.
Physical Demands & Working Conditions:
Ability to travel throughout NYC boroughs via public transportation. Ability to work from remote locations when necessary while maintaining confidentiality compliance. Must have access to email, phone, Zoom, and Teams. This role is primarily in-office.
