Job Description
Asian Community Care Management, Inc. is now hiring a Per Diem / Part-Time Health Home Care Coordinator. You will help clients understand their healthcare benefits, schedule doctor appointments, connect them with community resources (such as housing, food, or mental health services), and check in regularly to make sure they are receiving the care they need.
