The ideal candidate will play a key role in managing patient care and ensuring seamless communication between patients, families, caregivers, and staff.
Responsibilities:
Schedule and coordinate services for clients, families, caregivers, and providers. Manage a small caseload with attention to detail and responsiveness. Maintain ongoing communication with agency personnel, referral sources, and contracted partners to support daily operations. Utilize the HHAExchange scheduling system to manage patient services effectively. Participate in additional coordination activities as needed.
Qualifications:
A high school diploma is required; some college or business education is preferred. Minimum 2 years of experience in a Home Care setting . Proficiency with HHAExchange is required.
