Role:
As a Supervisor of Care Coordination, you will play a critical leadership role in guiding a team of care coordinators and ensuring the delivery of high-quality, patient-centered services. This position focuses on supporting social services and preventive care initiatives, with an emphasis on improving health outcomes for adult patients. You will oversee daily operations, manage caseload distribution, and provide clinical and administrative supervision to staff. In collaboration with multidisciplinary teams, you will coordinate comprehensive care plans that address both medical and social determinants of health. The role places a strong emphasis on prevention and patient education, including HIV prevention strategies such as PrEP and PEP, long-acting injectable (LAI) treatments, and broader preventive care services. Experience or familiarity with sexual health education and adult patient care is highly valued. Key Responsibilities:
As a Supervisor of Care Coordination, your responsibilities will include:
Team Leadership:
Supervise a team of Patient Navigators, Case Managers, and Supportive Case Managers, providing guidance, support, and performance evaluations.
Case Management:
Provide direct case management services, including intake, assessment, treatment planning, and ongoing reassessment.
Care Coordination:
Coordinate care with medical providers, behavioral health professionals, and other community resources to ensure seamless patient care.
Entitlements and Referrals:
Assist patients in accessing entitlements and referring them to appropriate services.
Documentation:
Maintain accurate and timely documentation of all patient encounters and activities.
Performance Management:
Monitor staff performance, provide feedback, and address any performance issues.
Quality Improvement:
Participate in quality improvement initiatives and contribute to the development of best practices.
Professional Development:
Attend conferences, trainings, and workshops to stay updated on the latest trends and best practices in care coordination.
Work Schedule:
This is a full-time position with evening hours. The schedule is structured to support operations across our Bronx, Brooklyn, and Chelsea sites.
