Non-Exempt (Hourly); Full Time Reports to: Manager, Practice Administration Direct report(s): None Work hours: 40 hours per week; Monday - Friday 8:30 AM-5:00 PM; very occasional evenings and weekends known well in advance Work location & format: 417 South Ave, Rochester, NY 14620; some offsite community outreach and/or community work via Mobile/Medical Outreach and Engagement (MOE) vehicle

Founded in 1993, St. Joseph's Neighborhood Center (SJNC) provides comprehensive, compassionate healthcare, mental health counseling, social work, and support services to individuals and families who are uninsured or face barriers to care. Each year, we serve roughly 1,500 members of the Rochester community through a holistic, person-centered model rooted in dignity, equity, and access. Our work is powered by a dedicated team of professionals and more than 450 committed volunteers who share a passion for service and community wellness.
Position Summary:
The Integrated Health Specialist supports SJNC's delivery of patient care under the supervision of licensed healthcare professionals. The Integrated Health Specialist performs clinical and administrative tasks within the scope of practice for unlicensed personnel as defined by New York State regulations and organizational policies. Discharge and Transition of Patients (15%) 5. Miscellaneous Duties (5%) Details of Key Job Responsibility Areas 1. Responsible for discharge and transition of care for all patients (15%)
  • Completion of discharge tasks in HER.
  • Send discharge letters to patients as appropriate.
  • Assist with transition to outside Primary Care Physician (PCP) if needed. 2. Assisting with the Financial Assistance (FA) program (25%)
  • Coordinate with the interdisciplinary team to determine who needs FA for continued care.
  • Meet with patients to answer questions about FA; complete and submit application and necessary documents to area hospitals or internal FA program.
  • Follow up and monitor the application through the process, documenting in the EHR, and notifying patient and Integrated Health Services team when assistance is approved. 3. Prescription Assistance Program (25%)
  • Coordinate with the medical department to determine who needs prescription assistance.
  • Meet with patients to answer questions about prescription assistance, and complete and submit applications to respective manufacturers.
  • Follow up and monitor the application through the process, documenting in the EHR, and notifying patient and Integrated Health Services team when assistance is approved.
  • Coordinate with appropriate team members for documentation of medications received and when picked up by the patient.
  • Monitor and collaborate with medical department on refill process. 4. Assist with engaging patients in care and treatment (30%)
  • Participate in the Integrated Health Services daily huddle.
  • Address patients' Social Determinants of Health (SDOH) needs as screened by the interdisciplinary team.
  • Utilize the UniteUs 360 Collaborative referral tool for all outside referrals.
  • Call and enroll all patients interested in SJNC services.
  • Complete annual recertifications with patients to determine fees for service.
  • Ensure all proper documentation and forms are accurately entered into EHR system.
  • Actively engage patients in their care and address any noncompliance issues with the patient.
  • May involve occasional visits to patients' homes as well as travel locally to attend service provider meetings for case conferences.
  • May occasionally operate Mobile/Medical Outreach and Engagement (MOE) vehicle MOE for community outreach and/or community engagement activities. 5. Miscellaneous duties (5%)
  • Stays informed of new community resources and programs for the SJNC patient.
  • Employees are accountable for meeting the performance standards of their departments and may be called upon to participate in required compliance audits, process improvement, and quality improvement plans.
  • Other duties as assigned. Requirements and Qualifications
  • Minimum of 1-year previous work experience in a community-based organization, or human services agency working with economically challenged populations strongly desired.
  • Bachelor's degree in Social Work, Public Health, Sociology, or related Human Services field; equivalent amount of experience in lieu of education may be considered. Experience using EHR preferred.
  • Bilingual Spanish highly desired.
  • Reliable transportation required - use of personal vehicle Preferred; if using personal vehicle in the scope of work, must possess liability insurance of $100,000/$300,000. Skills and Abilities

  • In compliance with New York State Department of Health (NYSDOH) infection control requirements for healthcare organizations, St.
