MENTAL HEALTH CARE COORDINATOR III DISTINGUISHING FEATURES OF THE CLASS
This is a professional who provides comprehensive care management in the context of a Health Home to coordinate all aspects of physical and behavioral health care; to include health promotion. The incumbent is responsible for assuring appropriate follow up is provided at points of transition such as: discharge from medical or psychiatric inpatient stays, or forensic facilities, or substance abuse rehab services; support to the individual (member) in obtaining services and developing community supports through referral and linkage to community and social supports; and will utilize information technology in the performance of duties. The incumbent will serve individuals with Severe and Persistent Mental Illness. A Mental Health Care Coordinator III is also located in the Children and Youth Care coordination unit. The incumbents of these positions will be required to provide care management and coordination for children and youth identified as experiencing Severe Emotional Disturbance and having the need for integrated care. This integrated care would be consistent with that which is required for the adult population and will be integral to the Children's Health Home delivery. A Mental Health Care Coordinator III also does related work as required.EXAMPLES OF WORK
(Illustrative only) Maintain a strong professional relationship with the Health Home provider network and beyond; Engage behavioral health lower need consumers in a strong working relationship and promote their engagement in the therapeutic services to meet their respective medical needs; Promote and encourage resiliency and recovery; Promotes wellness through assisting consumers and members in accessing an array of behavioral health, physical health, and social service linkages for those serves identified to be needed; Monitor mental status for fluctuations which may indicate risk for inpatient admission or use of ER services; Track inpatient admission and provide follow up upon discharge to prevent re•admission; Establish strong liaison activities with emergency, criminal justice, and community agencies which are often involved in crisis response; Participate in ongoing assessment of level of intervention (touch) with Health Home providers; Responsible for the development, implementation, and revision of a person centered care plan to meet the identified medical and behavioral health needs of the consumer;MENTAL HEALTH CARE COORDINATOR III

Obtains the necessary services to provide the care identified in the plan; Provide on-call coverage for the programs caseload as assigned in the program's on-call rotation; Participate in "Officer of the Day" rotation; Does related work as required.FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISITCS
Thorough knowledge of mental illness, chemical addiction, physical health, crisis intervention, and assessment; thorough knowledge of community agencies, facilities and services which can be utilized to promote wellness; ability to establish and maintain cooperative relationships with community providers and members; ability to communicate effectively both orally and in writing; ability to prepare and maintain integrated care plans in coordination with health home providers; possess qualities of dependability, initiative, and good judgment.MINIMUM QUALIFICATIONS
A. Graduation from a regionally accredited or New York State registered college or University with a Master's Degree in a human services field and one (1) year of relevant experience; or B. Graduation from a regionally accredited or New York State registered college or University with a Bachelor's of Arts or Science Degree in a human services field and two (2) years of relevant experience.ADOPTED:
