Make an Impact as a Care Coordinator The NewPath ChildFamily Solutions Care Coordinator is responsible for coordinating the implementation of the Plan of Care, including a Transition and Crisis Plan, through service delivery with providers and community resources. The Care Coordinator cultivates flexible, family-focused, community-based responsive services based on the High Fidelity Wrap Around model of care coordination.

Must successfully complete agency provided skill and competency-based training post hire. Experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development. Experience working with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child-serving systems (e.g., child welfare, juvenile justice, education). Must be a minimum of 21 years of age, have a valid driver's license and proof of current auto insurance with no more than 5 points on your MVR. Must be able to pass a criminal background check.
