The role ensures that every client leaves New Horizons with a safe, comprehensive, and individualized continuing care plan that supports long-term recovery, stability, and successful community reintegration. The Discharge Planner works closely with therapists, peer supporters, case managers, medical providers, sober living programs, community agencies, probation officers, family members, and referral sources to ensure continuity of care and eliminate barriers to successful discharge. Duties Discharge Planning Begin discharge planning upon admission and continually update the discharge plan throughout treatment. Maintain a discharge planning caseload for all assigned clients. Ensure every client has a completed discharge plan prior to discharge. Coordinate discharge meetings with clients, therapists, family members, and support systems when appropriate. Facilitate warm handoffs to the next level of care. Coordinate successful transitions from PHP to
IOP, IOP
to OP, and OP to community recovery supports. Ensure discharge plans meet Joint Commission, OMHAS, Medicaid, and agency standards. Continuing Care Coordination Schedule all aftercare appointments prior to discharge whenever possible.
Coordinate referrals to:
Individual therapists Psychiatric providers MAT providers Primary care physicians Peer recovery services Recovery housing Sober living programs Community support agencies Employment and vocational resources Educational resources Recovery community organizations Confirm receipt of referrals and transfer documentation. Recovery Support Planning Assist clients in developing relapse prevention and recovery maintenance plans. Encourage linkage to sponsors, recovery coaches, peer supporters, AA, NA, Celebrate Recovery, SMART Recovery, and other recovery support services. Coordinate family support and educational resources when appropriate. Assist clients in identifying healthy support systems and community connections. Experience and Qualifications Ideal candidates will possess the following skills and experiences: High School Diploma required. Associate degree or higher preferred in social work, counseling, human services, business administration, or a closely related field. Reliable transportation and a clean driving record are required. One year of experience in a substance use and/or mental health treatment environment required. Experience in behavioral health, addiction treatment, case management, discharge planning, or care coordination preferred. Strong organizational and communication skills. Proficiency with EMR systems and Microsoft Office. Demonstrates critical thinking, sound reasoning, and judgment. Effective communication skills and processes. Fosters a healthy and positive team approach with staff. Experience working with Medicaid populations. Ability to identify client needs that cannot be met by the counselor or agency and assist clients in utilizing the support systems and community resources available. Demonstrate knowledge of accepted principles of client record management and document appropriately. Ability to establish rapport, address, and assist clients in building and achieving measurable treatment goals and objectives. Ability to provide education and information to individuals and groups, through formal and/or informal processes, concerning their substance abuse and/or mental health issues. Demonstrate appropriate privacy practices and adherence to HIPAA and confidentiality policies when responding to inquiries for client information or copies of information. Demonstrated adherence to acceptable ethical and behavioral standards of conduct. Demonstrates the ability to prioritize tasks and responsibilities and complete duties in the allotted time.
PHYSICAL DEMANDS
Ability to lift up to 25 pounds. Ability to walk up and down stairs during emergency drills or situations.
