This role is essential in supporting clients as they navigate behavioral health challenges, crisis situations, and complex social‑service systems. The ideal candidate will demonstrate a strong dedication to client advocacy, ensuring individuals receive the resources, guidance, and support necessary to enhance their overall quality of life. Responsibilities Review assessments to identify client needs and develop individualized service plans. Provide crisis management support and intervention as needed. Facilitate access to community resources, including healthcare, housing assistance (such as Section 8), and childcare services. Collaborate with multidisciplinary teams to coordinate care and support for clients. Offer mentoring and guidance to clients, helping them set and achieve personal goals. Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies. Monitor client progress and adjust service plans as necessary to ensure effective outcomes. Advocate for clients within the healthcare system, including navigating Medicare processes when applicable. Qualifications Deploma or higher. CDCA Experience in behavioral health settings is highly desirable. Familiarity with motivational interviewing techniques to engage clients effectively. Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds. Excellent organizational skills and attention to detail in managing multiple cases simultaneously. Ability to work independently while also collaborating effectively within a team environment.
