This role is responsible for conducting in-depth, multi-dimensional assessments to evaluate physical, functional, social, and environmental needs, determine eligibility for Medicaid and waiver programs, and develop person-centered care plans. Key Responsibilities Determine eligibility for Medicaid and waiver programs based on assessment findings. Develop and implement person-centered care plans tailored to individual needs. Maintain accurate and timely documentation in compliance with state and agency guidelines. Collaborate with individuals, caregivers, healthcare providers, and community resources to coordinate care. Ensure adherence to
PASSPORT
standards and procedures throughout the assessment process. Qualifications Current licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed Social Worker (LSW). Minimum of one year of experience in home care for elderly and/or disabled adults, home health care, medical social work, geriatrics/gerontology, or approved substitutes as per ODA guidelines. Strong clinical judgment and excellent assessment skills. Knowledge of Medicaid, waiver programs, and

standards and procedures. Effective communication skills, both verbal and written. Ability to manage documentation and ensure compliance with state and agency requirements. Why Join Us?
