Job Description

Who are you? You are a caring professional who is dedicated to working with clients through all walks of life that includes but is not limited to homelessness, substance use, primary mental health diagnosis and/or co-occurring disorders, incarcerated population. You have a passion for making a difference in your local community by providing case management services through direct contact with clients, families and essential others in their natural environment. Under the guidance of the Nursing Supervisor, you will serve as a client advocate for our medical-only clients, acting in their best interest at all times. Attend treatment plan development and review meetings. You will also coordinate and monitor appropriateness of all services, examine and recommend new programs, and provide assistance in crisis situations to stabilize your clients. You will ensure adequate client care, provide consultative services and/or promote the coordination and development of mental health and/or substance abuse services within the community. Record client progress, monitor relapse plan and maintain necessary records and files. You will provide support, resources and linkage to services for your clients by maintaining a relationship with other agencies or external service providers as needed. You thrive working in a team-based environment. While working with the treatment team, you will maintain a strengths-based approach as you work with your clients and families to assist with the retention of dignity and the development of positive qualities in the client.
You have:
HSD/GED Previous experience in healthcare settings A valid driver's license and the ability to maintain auto insurance The willingness to travel to other FRC locations that we service, including Hancock County and surrounding areas CPR/First Aid Certification or willingness to obtain one and willingness to submit background check, drug screening and fingerprinting Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously Preferably Bachelor's degree in social services related field or Medical Assistant Certification Previous relevant experience working with individuals with primary mental health diagnosis and/or co-occurring disorders, SUD, or related populations Who are we?
