Primary Responsibilities:
1. Provide initial and ongoing social service to resident, families, and staff through room visits, staff meetings, and other interactions throughout the facility. 2. Perform all duties involved in resident advocacy. 3. Ensure the resident's psychosocial needs are being met. 4. Provide a continuing assessment of the resident care plan by direct contact with family members on a regular basis and maintaining a friendly, supportive relationship (1 to 1, family meetings). Notify the family/responsible party of scheduled care conferences, encourage them to remain involved in the care and life of a resident, and inform them of any change in the resident's psychosocial well-being. 5. Maintain proper documentation and perform general duties. 6. Conduct MDS's and assessments as needed. 7. Assist with planning the resident's discharge. 8. In charge of managed care updates for skilled residents. Who are we you may ask?
