Summary We are seeking a compassionate and dedicated Case Manager to join our team at Certus Healthcare Management. In this role, you will play a vital part in coordinating care for our residents, ensuring they receive personalized support tailored to their unique needs. Your contributions will directly impact the quality of life for those we serve. This position works out of our corporate office located in Mayfield Heights, Ohio. The typical work schedule for this position is Monday through Friday from 9am to 5pm. Start and end times have some flexibility and can be discussed during interview process. Salary will be discussed during the interview process. Responsibilities Develop and implement individualized care plans for residents based on their specific needs and circumstances. Conduct assessments to evaluate residents' health status and needs for services. Coordinate with healthcare providers, families, and community resources to ensure comprehensive support. Provide crisis intervention and support to residents facing challenges or emergencies. Facilitate communication between residents, families, and staff to promote understanding and collaboration. Monitor progress and adjust care plans as necessary to achieve optimal outcomes. vaccines Requirements Bachelor's degree in social work, psychology, or a related field preferred. Experience in behavioral health or social work is highly desirable. Strong knowledge of Medicare regulations and community resources. Excellent communication skills for effective interaction with residents and families. Ability to conduct motivational interviewing techniques to encourage resident engagement. Experience in crisis intervention is a plus.
