Role Job Summary:
The PACE Care Advocate serves as a primary point of contact for participants and caregivers, ensuring their needs, preferences, and concerns are consistently communicated and addressed through the Interdisciplinary Team (IDT). This role functions as the program's frontline advocate in the home, facility, and community, supporting participant engagement, identifying service gaps, and promoting coordinated, participant-centered care. The Care Advocate plays a key role in strengthening relationships, improving service delivery, and ensuring a seamless experience across clinical and non-clinical services.
Education, Licenses, Certifications and Experience:
High school diploma or equivalent required; associate's or bachelor's degree preferred Experience in healthcare, social services, case management, or community-based programs preferred Experience working with older adults and medically complex populations preferred Share job details to PACE Care Advocate Valir
