Job Description
Resident Care Coordinator/Scheduler Schedule:
Tuesday-Saturday 10 am-6 pm. A premier Senior Living community in West Linn, Oregon is looking for a great and talented Resident Care Coordinator/Scheduler. A great candidate will be a Med Tech/ Caregiver as you will be helping on the floor and or covering shifts if needed. Have open availability to work 10 am-6 pm.Purpose:
Resident Care Coordinator/Scheduler is responsible for managing all day-to-day operation of the residents and leading their team to the highest possible standards. They are additionally responsible for achieving and maintaining occupancy to full capacity while maintaining high resident satisfaction and quality resident care and services with the Executive Director or in their absence. The Resident Care Coordinator/Scheduler is also expected to ensure compliance with applicable State and Federal requirements, promote a positive reputation/image and ensure smooth operation within the budgeted expectations for the organization, in coordination of the Executive Director.Duties & Responsibilities:
- Assist in the planning, development, organization, implementation, evaluation and direction of all resident services and nursing policies and standards.
- Complete a pre-admission evaluation on resident applicants.
- Review resident service plans at least monthly and provide a service plan conference not less than quarterly or in accordance with policy and regulatory compliance. Ensure that service plans meet the goals, needs and services are being addressed in a manner appropriate for the resident or his/her representative.
- Provide documentation of resident services and response, changes in resident condition and service plan changes consistent with established policy and procedure.
- Develop and maintain a good working rapport with inter-department employees, other agencies, and businesses to assure that the services and activities can be properly maintained to meet the needs of the residents.
- Assure employees, residents, families, and guests are informed and adhere to established policies and procedures while still influencing an atmosphere of warmth, personal interest, positive emphasis and calm environment throughout the Community.
- Hire, and evaluate employees as well as make recommendations to the Health Services Director or the Executive Director. Assist in setting standards for employees. Meet with Department employees, on regularly scheduled basis, to assist in identifying and reinforcing positive performance and areas of opportunity to enhance skills and/or service delivery.
- Assure staff, including private duty caregivers, follow established policies, procedures, and philosophy.
- Review complaints and grievances from residents and their families as well as staff and making necessary reports to the Executive Director. Communicates departmental status to Executive Director on a regular basis ensuring he or she is fully aware at all times of any potential issues so they can be addressed proactively.
- Plan and execute in-services and infection control as needed per regulations. Provide additional training for all staff related to the care of the residence.
- Provide regularly scheduled vital sign monitoring for residents who request or require such.
- Assure a stock level of medications, medical supplies, equipment, etc., are always maintained at adequate levels. Requisition equipment and supply needs as required in accordance with department budget and Residence policies. Ensure medication system and resident health records are used appropriately and adequately.
- Prepare and administer medications as ordered by the physician.
- Assure the medication room(s) is maintained in a clean and safe manner for resident comfort and convenience by assuring necessary equipment and supplies are maintained to perform such duties and services.
- Work with the consulting pharmacy to promote an optimal medication regime for each resident. Assist with scheduling pharmacy in-services to maintain education on medication management for medication aid staff.
- Notify resident's physician, as well as family member or personal representative, when a resident becomes acutely ill, is injured, etc., as directed by Residence policy. Provide liaison with physicians, physician's assistants, or nurse practitioners through regularly scheduled review of resident service plans and physician orders.
- Work with the Residence consultants as necessary and implement recommended programs, etc. Plans, communicates, and coordinates with medical and/or other providers concerning the health care needs of the residents as needed and/or requested.
- Evaluate emergencies and determine emergency measures to be used. Notify the physician, record, and document orders.
- In cases of death, the Resident Care Coordinator/Scheduler may call the funeral home as indicated in the resident information and as otherwise outlined in the policy and procedures of the Residence.
- Make decisions regarding health care of residents within the community and assure the highest quality care of the residents and the morale of the staff.
- Complete accident and incident reports for residents when appropriate. Notify physicians or designated representative if required or requested by the resident.
- Attend and participate in professional association activities and programs to assure knowledge of current regulations and guidelines as well as professional standards. Make recommendations regarding policy and procedures enhancements to the Executive Director.
- Complete necessary forms/charts upon the residents' admission, discharge, transfer, etc. File in the resident's chart or forward to the appropriate department.
- Provide orientation for new residents, their families, and staff.
- Assure a multi-disciplinary approach to resident services.
- Provide creative approaches which involve the resident, their appointed representative, etc. in resolution of problems or personal care issues
- Create the schedule for the care staff. Ensure all shifts are covered.
- Participate in Department Head meetings.
Other Responsibilities:
- Maintains current knowledge of regulations for state licensure of Residence.
- Updates Case Manager of resident status.
- Ability to work with wide range of persons and needs.
- Ability to assist in marketing the Community to meet occupancy expectations per budget.
- Reports all incidents and accidents, both resident and employee.
- Attends and participates in training, in-services, and staff meetings.
- Other duties as assigned.
Minimum Requirements:
- Experience in Senior care, or equivalent education and experience as approved by the community.
- Ability to type at least 45 words per minute and have computer expertise at a level necessary to accomplish this job.
- Experienced in personnel management.
- Understanding of budgeting, financial record keeping and reporting, appropriate State and Federal regulations, operational systems and procedures and must be able to apply understanding to ensure the smooth operation of the Residence.
- Maintain current driver's license.
- Ability to travel by motor vehicle.
- Ability to read and comprehend verbal and written communication and write at a level necessary to accomplish job. Ability to communicate effectively with others verbally and in writing.
- Compassion for the elderly and sick. Ability to relate to the elderly in a positive manner.
- Self-motivated with the ability to work with little or no supervision.
- Ability to work flexible hours to meet the requirement of the job.
- Maintain reliable attendance and notify supervisor if late or unable to work.
- Organizes and utilizes time efficiently with an ability to appropriately set priorities to accomplish assigned tasks in a timely manner.
- Displays professional, mature behavior and attitude in speech and action. Demonstrates consistently pleasant demeanor and tone of voice with others.
- Possess a high degree of interpersonal relationship skills and demonstrates the capability of relating to a variety of people and personalities.
- Keeps information confidential.
- Keeps work area neat and clean and helps maintain the appearance of the Residence.
- Ability and willingness to learn quickly.
- Looks for ways to improve skills of self, staff, and systems.
- Accurate, dependable, and thorough in all tasks. Ability to perform assigned tasks in neat, thorough, and timely manner.
- Capable of performing the essential functions of this position.
- Maintain current certifications in CPR and First Aide.
