Overview The Ophthalmic Triage Coordinator serves as a critical link between the front desk, clinical team, and referring providers. This role is responsible for coordinating patient care activities, managing triage communications, and ensuring timely follow-up on both internal and external patient care needs. Ophthalmic Triage Coordinators triage incoming patient concerns, initiate and manage physician communications within MDI, and provide patient instructions as directed by providers. They play a key role in prioritizing urgent versus routine needs, facilitating appropriate scheduling, and ensuring continuity of care. In addition, this role supports referral management by monitoring incoming physician communications, coordinating outbound referrals, and confirming completion of external care and documentation review by PRS physicians. The Ophthalmic Triage Coordinator also supports patient access and compliance initiatives through pre-registration, outreach, and follow-up efforts.
Duties/Responsibilities:
Patient Triage & Communication Receive and assess incoming patient calls to obtain symptoms and identify care needs Document all triage interactions thoroughly in the patient's medical record Initiate and manage MDI communications with PRS physicians for clinical guidance Communicate physician instructions to patients and coordinate same-day or urgent appointments as needed Respond to patient questions and provide guidance regarding medications and care instructions within scope Referral & Care Coordination Monitor and manage incoming referring provider communications and fax folders Prioritize referrals based on urgency and coordinate timely scheduling Manage outbound referrals, including: Initiating referrals through MDI tasks Following up to confirm patients were seen Ensuring consult notes are received and reviewed by PRS physicians Research and coordinate out-of-area referrals for patients relocating or traveling Patient Access & Pre-Registration Conduct pre-registration for: New patients Established patients not seen within the past 3+ years Accurately enter and update: Medical, surgical, and ophthalmic history Medications and allergies Demographic and insurance information Operational Support Maintain consistent communication with Front Desk leadership regarding workflow issues, barriers, or concerns Perform additional duties and provide coverage as assigned Required Skills & Competencies Strong knowledge of electronic medical records (2+ years required) Excellent verbal and written communication skills Strong interpersonal and customer service skills High attention to detail and organizational ability Effective time management with the ability to prioritize competing demands Strong critical thinking and problem-solving skills Ability to work independently and collaboratively in a fast-paced environment Proficiency in Microsoft Office or related systems Education & Experience High school diploma or equivalent required 3+ years of ophthalmic assistant/technician experience required (retina preferred) Prior experience triaging patient calls in an ophthalmology setting strongly preferred Physical Requirements Prolonged periods of sitting and computer use Ability to lift up to 15 pounds occasionally
